Core identity

MC + USDOT numbersOn letterhead or a one-pager. Match FMCSA exactly (legal name, not a nickname).
Operating authority letter / snapshotCommon/contract (and broker if applicable). Print or PDF from FMCSA / your registration pack.
W-9 (current year)Legal name + EIN/SSN as on file with the IRS. Signed and dated.

Insurance

Certificate of insurance (COI)Auto liability + cargo limits brokers expect for your equipment. Ask your agent for a fresh cert.
Additional insured / waiver readyMany brokers want to be listed. Have your agent’s email ready so SDS/you can request the same day.
BIPD / cargo on FMCSAConfirm insurance shows on FMCSA before you chase loads — Carrier Lookup helps.

Factoring / pay

Notice of Assignment (if you factor)Factor’s NOA + remittance instructions. Brokers need this before they will pay the factor.
Factoring company intro letterWho funds you, contact for credit, and payment terms — reduces “who do we pay?” delays.
Voided check / banking (if direct pay)Only if you do not factor. Never send this unencrypted to random emails.

Ops contacts

ELD / tracking contactProvider name + who can share location for in-transit updates.
Dispatch + after-hours contactsOne phone and one email that actually answer. Put them on every rate con reply.
Equipment list (optional but strong)Unit #, year, type, door size / reefer, hazmat if any. Speeds carrier setup on big brokers.
